Proformas
Data from operations can be entered directly into the NJR database. Where this is not feasible, proformas (paper forms) can be used to record data prior to electronic submission:
- Proforma H1, for primary (complex) hip operations.
- Proforma H2, for single stage revision, hip revisions and hip excision arthroplasty
- Proforma K1, for primary (complex) knee operations
- Proforma K2, for single stage revision, knee revisions, knee conversions to arthodesis and amputation.
Please be sure to complete each relevant section of the proforma for each procedure. Please do not return completed MDS (minimum data set) proformas to the NJR Centre, these should instead be made available to the hospital staff who are able to input NJR data.
Release history
The NJR published the most recent versions of its MDS proformas in November 2007 to coincide with updates to the NJR MDS. The changes to the proformas take into consideration the reduction in the number of fields being collected, the increase in the number of mandatory fields and user feedback.
